Authorized Physician User Approval

In order to add an Authorized Physician User to the University of Alabama at Birmingham (UAB) Radiation Oncology license # 314, Oncology Nuclear Medicine license # 237, Nuclear Medicine Division license # 313, and/or the Particle Accelerator registration PA-105, training and experience must be documented according to the requirements of Chapter 420-3-26 of the Rules published by the Alabama Department of Public Health, Office of Radiation Control.  The training, experience, and preceptor statement requirements of the State of Alabama Department of Public Health, Office of Radiation Control, for each category of radioactive materials or radiation use in humans are given in the document entitled: Authorized Physician User Training and Experience RegulationsPDF file*

To add an Authorized Physician User to one or more of the UAB radioactive materials or radiation use in humans licenses # 237, # 313, and/or # 314, fill out the forms for all categories that approval is sought for radioactive materials or radiation use in humans [(45), (48), (52), (56), (57), (58), (59), (60), (68), (69), (70), (72), and/or (89)].  To add an Authorized Physician User to the Particle Accelerator registration PA-105, obtain documentation of training and experience as required by the old State of Alabama regulations 420-3-26-.07(75), dated December 18, 1996, which are given in the last two pages of the above document.  Send all required documentation, preferably by e-mail attachment, to: smp@uab.edu.  Alternatively, hard copies can be sent by Campus Mail to the following address:

The Radiation Safety Officer verifies that all required documentation of training, experience, and adequate preceptor statements have been submitted for all categories that approval is sought as an Authorized Physician User of radioactive materials or radiation in humans.  The Radiation Safety Officer will request additional documentation if necessary.  After verifying that all necessary documentation has been submitted, the Radiation Safety Officer forwards the documentation to the members of the Subcommittee for Human Use (SCHU) of the Radioisotope and Radiation Safety Committee (RRSC) for consideration.  Additional information may be requested by the members of the SCHU.  In order for a physician to be approved as an Authorized Physician User of radioactive materials or radiation in humans in a category of human use, a majority of the members of the SCHU must vote to approve the physician as an Authorized Physician User in that category.  If you have any questions, please contact the Radiation Safety Officer for UAB at brin2929@uab.edu or 934-7418.
