UAB Employee Occupational Health Program

Eligibility

This program is designed for UAB Employees who:

  • Have direct contact with animals, their viable tissues, body fluids, wastes or living quarters.  This includes, but is not limited to, Animal Care Staff, Investigators, laboratory staff, and some Maintenance and Environmental Services personnel.
  • Work in the laboratory and have direct contact with material of human origin.
  • Have direct contact with material capable of causing disease or injury in humans.
  • Have direct contact with raw sewage through plumbing activities.
  • Are exposed to excessive levels of noise (>85 db).

Individuals not employed by UAB, but who will be conducting work with any of the above at UAB, must provide proof that their proposed work/potential exposure at UAB along with their medical history has been reviewed by their personal physician.  A clearance document indicating the above, along with any work restrictions, must be signed by the physician on his/her office letterhead and provided to UAB Employee Occupational Health along with a copy of the Health Screening Questionnaire prior to being granted access to the animals or the animal facility.  Examples of those ineligible for the UAB Employee Occupational Health program include:

  • Non-paid students
  • Volunteers
  • Individuals from private companies conducting work at UAB
  • Visiting Scientists
  • UAB Hospital Employees (should be covered by the Hospital Employee Health Program)

How Do I Enroll?

  • Click on the appropriate link below for either the Initial Health Screening Questionnaire if you are enrolling for the first time or Health Screening Update if your job-related exposures change or you are due for reevaluation.
  • Print and complete the questionnaire.
  • Send the completed questionnaire in a confidential envelope to:

  • Once your completed questionnaire is received by the Nurse Coordinator and logged into the tracking system, you are enrolled and/or updated.
  • Your completed questionnaire will be reviewed by Occupational Health Professionals.
  • You may be contacted by the Nurse Coordinator if further evaluation is deemed necessary.

Frequently Asked Questions

  1. Who will see the information on my questionnaire?
    All questionnaires should be submitted in “Confidential” envelopes. All submitted information submitted will be treated as confidential and will only be reviewed by Occupational Health Professionals.
  2. I am a volunteer or visiting scientist. Can I enroll in this program?
    Only paid UAB employees are eligible for this benefit.
  3. I will be traveling abroad. Can I get immunizations for international travel through this program?
    Immunizations will only be provided if they are required or recommended for the work performed at UAB.
  4. What is the status of my animal protocol approval?
    All questions regarding specific animal protocols should be directed to the IACUC Office at 4-7692 or 4-7880.
  5. I am a PI and have completed the questionnaire and am thus enrolled in the Occupational Health Program. Shouldn't my animal protocol be approved by now?
    All personnel listed as involved on the Animal Use Review Form must be enrolled in the Occupational Health Program prior to protocol approval. Other factors (i.e., providing answers to all questions posed by the IACUC, satisfying training requirements, standard processing times, etc.) may be involved.
