 Services A variety of services are provided by OH&S Employee Health. Those offered to you are dependent upon the potential risks posed by the work you conduct at UAB. You may formally decline services that are offered to you, as long as they are not deemed mandatory for the work you will be performing (e.g., if you will be working with NHP, your TB status must be assessed annually). Once your Enrollment Form is received by OH&S Employee Health, the Occupational Health Professionals will review the work description and your medical history to determine if services should be offered to you for your protection. If the Employee Health Professionals determine that an immunization or screening is warranted, you will be contacted and asked to schedule an appointment. Once you have scheduled your appointment, it is your responsibility to remember when it is and to show up for the appointment. You will not be called and reminded. If you fail to show up for a scheduled appointment, your Employee Health Status will not be updated, and you must call to reschedule. If you miss two appointments, your supervisor may be notified. We value and respect your time and expect the same courtesy in return.
