  • Click on the link for the Employee Health Enrollment Form PDF file, if you are enrolling for the first time, or if your job-related exposures change, or you are due for reevaluation.
  • Print and complete the form.
  • Send the completed form in a confidential envelope to:

  • Once your completed form is received by the Nurse Coordinator and logged into the tracking system, you are enrolled and/or updated.
  • Your completed form will be reviewed by Occupational Health Professionals.
  • You may be contacted by the Nurse Coordinator if further evaluation is deemed necessary.

How to Update Enrollment

  • Click on the appropriate link for either the Enrollment FormPDF file, if your job-related exposures change or you are due for reevaluation, or the Annual TB Form PDF file. (If you are not sure which is due, you may determine your enrollment status by logging into BlazerNET, select the Courses and Training tab, and review your Training and Requirements dashboard)
  • Print and complete the questionnaire.
  • Send the completed questionnaire in a confidential envelope to:
